Qualified Birth or Adoption Withdrawals

If you have a new child, or legally adopt someone (other than the child of your spouse) who is either (i) under the age of 18 or (ii) cannot physically or mentally take of himself/herself, you may be able to take a qualified birth or adoption withdrawal from your account, of up to $5,000 per child or adoptee.

A qualified birth or adoption withdrawal can only be taken within a year period of the date of birth or legal adoption. Participants who have terminated employment with the Company are not eligible to take a qualified birth or adoption withdrawal. If you are receiving severance installment payments under a separation plan or severance plan of the Company, you will not be treated as having "terminated employment” for this purpose until the end of your separation period under that plan. Qualified birth or adoption withdrawals will be taken pro rata from vested balances of all available sub-accounts in your Plan Account, unless you request that your withdrawal be taken from certain sub-accounts by calling Fidelity at 1-800-835-5095.

If you take a qualified birth or adoption distribution, you may be able to (but are not required) repay the amount of the distribution to your Plan account at a later date. To make the repayment, you must meet the requirements for making a rollover contribution to the Plan.
